Cardiac MRI evaluation of aortic biophysical properties in paediatric Turner syndrome

Background:
Aortopathy in Turner syndrome is associated with aortic dilation, and the risk of dissection is increased when the aortic size index is ≥ 2–2.5 cm/m2. We evaluated the aortic biophysical properties in paediatric Turner syndrome using cardiac MRI to determine their relationship to aortic size index.
Methods:Turner syndrome patients underwent cardiac MRI to evaluate ventricular function, aortic dimensions, and biophysical properties (aortic stiffness index, compliance, distensibility, pulse wave velocity, and aortic and left ventricular elastance). Spearman correlation examined correlations between these properties and aortic size index. Data was compared to 10 controls.
Results:Of 25 Turner syndrome patients, median age 14.7 years (interquartile range: 11.0–16.8), height z score −2.7 (interquartile range: −2.92 – −1.54), 24% had a bicuspid aortic valve. Turner syndrome had increased diastolic blood pressure (p < 0.001) and decreased left ventricular end-diastolic (p < 0.001) and end-systolic (p = 0.002) volumes compared to controls. Median aortic size index was 1.81 cm/m2 (interquartile range: 1.45–2.1) and 7 had an aortic size index > 2 cm/m2. Aortic and left ventricular elastance were greater in Turner syndrome compared to controls (both p < 0.001). Increased aortic size index correlated with increased aortic elastance (r = 0.5, p = 0.01) and left ventricular elastance (r = 0.59, p = 0.002) but not aortic compliance. Higher ascending aortic areas were associated with increased aortic compliance (r = 0.44, p = 0.03) and left ventricular elastance (r = 0.49, p = 0.01).
Conclusion:Paediatric Turner syndrome with similar aortic size index to controls showed MRI evidence of abnormal aortic biophysical properties. These findings point to an underlying aortopathy and provide additional parameters that may aid in determining risk factors for aortic dissection.

This review offers an update on research conducted with FinnTwin12 (FT12), the youngest of the three Finnish Twin Cohorts. FT12 was designed as a two-stage study. In the first stage, we conducted multiwave questionnaire research enrolling all eligible twins born in Finland during 1983–1987 along with their biological parents. In stage 2, we intensively studied a subset of these twins with in-school assessments at age 12 and semistructured poly-diagnostic interviews at age 14. At baseline, parents of intensively studied twins were administered the adult version of the interview. Laboratory studies with repeat interviews, neuropsychological tests, and collection of DNA were made of intensively studied twins during follow-up in early adulthood. The basic aim of the FT12 study design was to obtain information on individual, familial and school/neighborhood risks for substance use/abuse prior to the onset of regular tobacco and alcohol use and then track trajectories of use and abuse and their consequences into adulthood. But the longitudinal assessments were not narrowly limited to this basic aim, and with multiwave, multirater assessments from ages 11 to 12, the study has created a richly informative data set for analyses of gene–environment interactions of both candidate genes and genomewide measures with measured risk-relevant environments. Because 25 years have elapsed since the start of the study, we are planning a fifth-wave follow-up assessment.

Summary
Introduction
Terrorist incidents and severe natural disasters have focused concern on preparedness and response activities to enhance psychological and social adjustment in the aftermath of extreme events and to minimize maladaptation over time. Borrowing from a growing body of work describing the characteristics that promote resilience in individuals, the concept of community resilience has emerged as a community-level construct to foster individual, family, and community adaptation. The likelihood of unanticipated terrorist events and the possibility of massive infrastructure destruction associated with major disasters increase the importance of community resilience strategies.
Relatively little is known about what constitutes a resilient community or about the factors, conditions, and processes that promote it. In this paper, we (1) describe the construct of community resilience in the context of disasters, (2) propose a set of contributing factors, (3) identify potential barriers, and (4) make recommendations for enhancing community resilience. The paper does not constitute a formal review of the literature on community resilience.
Definitions
A discussion of community resilience requires consensus in our understanding of several key terms including “community,” “disaster,” and “resilience.”
Community
The notion of community has been widely discussed in both the social science and health literatures (Cohen, 2003; Institute of Medicine, 2003; Jewkes and Murcott, 1996). Depending on context, a variety of conceptualizations and components have been described. Traditionally, a community refers to people, organizations, structures, and systems in close geographic proximity and with physical boundaries and borders (e.g., a town).

One of the daunting aspects of studying the Middle East is the confluence of several, often unrelated, languages, each with its own alphabet. Having gained some control over these, the student then comes up against a jumble of calendars. While many of these complications are lessened when focusing on a particular region or period of time, the cosmopolitan nature of the Ottoman Empire evolved a dense fabric of interwoven languages (Persian, Turkish, and Arabic) and of calendars, which were not only in use alongside each other, but were blended to create new subspecies! Handbooks and concordances have existed for a long time to enable the scholar to translate solar, lunar, agricultural, and urban time reckonings into modern calendars. We are most familiar with the standard Gregorian/Hijra guides, as Europeans were once deeply involved in concordances of Old Style (Julian) and New Style (Gregorian) conversion tables.
The Ottoman fiscal calendar is one of the more peculiar challenges which must be attended to, in order to control adequately the immense store of documents from the last centuries of the Turkish Empire. This calendar was employed particularly in the State’s fiscal and trade sectors; hereafter it is identified by the code SM, for sene-i-maliye, the fiscal year. It is a solar calendar, first put into use in AD 1676, and adopted by more areas of trade and administration until it becanle the official standard calendar of the empire in AD 1839. The supremacy of SM usage then lasted until AD 1917, when it was first modified to accord with Gregorian NS reckoning over Julian OS. Finally, SM usage was discontinued entirely in December of AD 1925, and replaced by the “Western” Gregorian calendar.
